What happens is that Sheldon persuades Leonard to spend days on end wearing an excruciatingly irritating sweater. Why, you may wonder? Since ‘itchy brain’ is a made-up condition that Sheldon invented. Funny, huh? Not really, though. Viewers grimace as Leonard suffers, but that’s not the worst part.
The episode’s conclusioncontains the real betrayal. Sheldon admits that the whole affair was a cruel joke, a test to see if Leonard could tolerate suffering, once his skin starts to resemble a battle zone. Brutal? Without a doubt. Everything that made Sheldon lovable was undercut by this cruel joke, including his terrible social skills and innocent childhood. All of a sudden, he came across as cunning and manipulative.

You see, Sheldon’s growth was his attraction. In spite of his eccentricities, he gradually gained relatability. ‘The Itchy Brain Simulation’ completely ignored that advancement. And it makes perfect sense why viewers of the series were freaking out.
Fortunately, by the end of the show, Sheldon had mostly gained his redemption. However, ‘The Itchy Brain Simulation’ acts as a reminder that even the most adored characters are capable of making perplexing choices. Plus, some jokes just give you a bad rash, let’s face it.
