A subsection of theDiablo 4community has been actively investigating the existence of a secret cow level, and they’ve just made a major breakthrough.
For those unfamiliar with the lore, the cow level originated as an online hoax in the late ‘90s. A Diablo player claimed that if you clicked on a specific cow several times, a red portal would open, allowing you access to a secret level. A photoshopped image was used to support the false claim.

The hoax became so popular that Blizzard included a secret cow level in Diablo 2 and Diablo 3. A part of the joke is that Blizzard never acknowledges the existence of any secret levels. Naturally, the community then expected to find a secret cow level in Diablo 4.

The search has been ongoing since Diablo 4’s release and the community has made several breakthroughs. With the release of season two, another major part of the puzzle has been uncovered.
The process begins by killing 666 cows around Sanctuary. As explained by Diablo streamerRaxxanterax, only the last cow kill is significant. The last cow slain will drop one of three specific quest items. The item is determined by which area of the map the final cow is killed in. After obtaining one of the items, a timer will begin, locking you out of obtaining either of the other two quest items.

While there was a belief that the timer is 666,666 seconds, this has been allegedly debunked. Either way, you’re able to use separate characters to obtain each item and transfer them to a single character. Blizzard has currently disabled trading due to a new duplication glitch, however.
After gathering all three quest items, you can travel to the cow shrine in Ked Bardu. Here, you can drop all three items in the fountain to “purify” them. Once you have all three purified items, you will be granted a key. This key opens a cellar in the far east of Scosglen which contains 28 cows and human corpses. If you kill all 28 cows, you get a stamina potion. This is a reference to Diablo 2’s Cow King, a secret cow level boss that drops stamina potions.

The next piece of the puzzle hasn’t been discovered yet. The dominant working theory is that something specific needs to be done while under the five-minute stamina buff from the potion. While there’s a chance that Blizzard has yet to add the final step of the puzzle, everyone is hopeful that the mystery is close to being solved. The bulk of this information was discovered thanks to the efforts of the ‘Not Finding a Cow Level’ Discord community.
