The first option would be that since the government doesn’t know the full limitations of Extremis, they would fill it up with Gamma rays (like what happened with Bruce Banner inThe Incredible Hulk).
The other option would be that General Thaddeus Ross accidentally gets exposed to Gamma rays and turns into the Red Hulk. This could potentially mean that we could get a cameo of Harrison Ford as the Red Hulk inCaptain America: Brave New World.
